Ms. Espinosa joined the OSH Investigation Team composed of DOLE 9 LLCO Sheryl Joaquin and ECC 9 Focal Jerrick Go, in response to an accident leading to fatality of 1 worker and critical hospitalization of 1 worker in Permex Exporter and Producer Corporation in Ayala, Zamboanga City.

Ms. Espinosa assisted in the assessment and inspection and deficiencies on OSH were noted. Recommendations were forwarded and for strict compliance.
