Occupational Safety and Health Standards Investigation

The DOLE RO5 QRP composed of DOLE, OSHC and ECC conducted an OSH Investigation at Chase Distribution, Inc. to determine the existence of imminent danger following an accident resulting to death of a worker based on a report or information received by the Regional Office. The team performed inspection on the scene of the accident with the assistance of the representative of the employer and conduct individual and group interview to selected employees to gather facts about the incident. After the conduct of Investigation, an NR was issued specifying the deficiencies in OSHS.
